Why did the document I try to submit get rejected?

The document must be a Medicaid, SNAP or WIC document and it CANNOT be a physical benefits card or ID card.

The document MUST contain the following:

• Child's name
• Current date of enrollment within 12 months 
• The name of the benefit 

The document CANNOT be a physical card.
