What do I need to fill out my application?

You will need the following information before you begin. Have it ready before you open the application.

• Qualifying document for each child 
• Youth sports organization (YSO) name. Do NOT abbreviate.
• Sport your child is playing
• Name of a contact at the organization (coach, owner, director, etc.)
• Organization contact's phone number
• Organization contact's email address
• Link to the organization's website or social media page
• Registration start and end dates
• Total registration cost for the season
• Your grant request amount ($150 or less)
